REACH Community Health Centre (Commercial and Napier St., PCN 2)
REACH Community Health Centre is an interdisciplinary team of medical professionals that offer a variety of health and social services including family medicine, dental services, pharmacy, counseling, registered nurses, social work services, and a Multicultural Family Centre. Embedded within the Community Health Centre there is a wide range of allied health supports and programming to attend to social determinants of health.
There are 8 Family Physicians (6.5 FTE) and 2 Nurse Practitioners (1.0FTE) currently working at this CHC, and there is an approximate 1:1 ratio of MOA to FP. There are 2 counsellors and 1 social worker on site daily from 9am-5pm to provide support, in addition to a full-time operations manager. There is active mentorship available through peer group support and weekly meetings. Opportunities for 1:1 mentorship can be negotiated.
Each physician occupies 1 exam room per shift, and the CHC has a total of 9 rooms. There are communal areas for team and informal team interactions as well as a group room, kitchen and outdoor patio for relaxation. Parking is free within 2 blocks of the office.
The provider will see an average of 15-20 patients a day, consisting of medically and psychosocially complex care including addictions care. Addictions care patients make up approximately 5-10% of the patient panel but account for a larger proportion of the clinic’s monthly visits, however the clinic serves full-spectrum family medicine from cradle to grave. There may be house calls for homebound patients, as well as early prenatal care up to 20 weeks, however there is no obstetrics or 3rd trimester care. No inpatient hospital work required.
The incoming FP will negotiate their set weekly schedule with the CHC, and there may be some flexibility in weekly scheduling thereafter, however this will be constrained by room availability. One day a week permanent staff/MRP’s will perform Telehealth, but all new locums are always required on site.
Remuneration is in the form of sessional pay, with a patient panel focus on addictions and complexity.
Qualifications include:
Physician registered (Full) with the College of Physicians and Surgeons of B.C.
Certification with the College of Family Physicians of Canada.
Commitment and ability to implement REACH’s strategic goals, mission, vision, and values.
A background in working with youth, mental health, addictions, fragile families, refugees, and frail elderly desired.
Current methadone exception and Suboxone certification or willingness to obtain same.
An awareness of the impact of colonization on Aboriginal peoples and understanding of cultural competency as a care provider.
Ability to speak more than one language is an asset.
Experience or training in delivering medical education an asset.
Experience or training in primary care research and policy an asset.
