This job posting is for a permanent part-time position (not locum). Now is the time to augment your family practice with a long term care (LTC) panel to provide the challenging, diverse, and impactful work of caring for the frail elderly.
Adanac Park Lodge is a 73-bed long term care home currently being supported by two Long Term Care Initiative (LTCI) practitioners. One practitioner is leaving and therefore an additional practitioner is required to assist with the resident caseload. In addition to LFP or fee-for-service payments, practitioners have the opportunity to join the LTCI which includes incentive payments for: mentorship and care home onboarding, participation in LTC-specific events and projects, and participation in the after-hours care program. Joining the LTCI represents a formal commitment to the care home and meeting the five best practice expectations which include: regular proactive visits; on-site medication reviews; attendance at care conferences; involvement in 24/7 on call; and completed documentation in charts.
Practitioners working in LTCI care homes will work with an interdisciplinary care team (physicians, nurse practitioners, nurses, allied health, and the care home leadership team) to provide holistic resident-centered care. If you are new to long term care, there is a strong community of practice that will help build your skill set and confidence.
