Lakeview Care Centre

Lakeview Care Centre is a 177-bed long term care home currently being supported by four Long Term Care Initiative (LTCI) practitioners. One practitioner is leaving and therefore an additional practitioner is required to assist with the resident caseload. Practitioners working in LTCI care homes will work with an interdisciplinary care team: physicians, nurse practitioners, nurses, allied health, and the care home leadership team. If you are new to long term care, there is paid mentorship available as well as a strong community of practice to build your skill set and confidence. In addition to fee-for-service payments, practitioners who join the LTCI program also qualify for incentive payments if they meet the best-practice expectations in their care home work.
