What you will be doing:
- Conduct thorough medical assessments and develop personalized care plans for clients.
- Provide ongoing preventative care, chronic disease management, and wellness coaching.
- Collaborate with a multidisciplinary team to ensure seamless, coordinated care.
- Build long-term relationships with patients, supporting them on their journey to optimal health.
- Utilize advanced diagnostic tools and testing to support clinical decision-making.
- Educate clients on preventative strategies, nutrition, fitness, and lifestyle interventions.
- Stay up to date with the latest medical advancements and incorporate evidence-based practices.
- Maintain accurate and comprehensive patient records within the electronic medical system.
You have:
- Licensed & in good standing (or eligible for licensure) to practice in British Columbia.
- Certification in Family Medicine preferred.
- Experience in preventative medicine, concierge healthcare, or wellness-focused practice is an asset.
- Strong commitment to patient-centered care and health optimization.
- Excellent interpersonal and communication skills.
- Ability to work collaboratively within a multidisciplinary team.
