Community Longitudinal Family Physicians
If you are a community longitudinal family physician (as defined in the FPSC Preamble), you can submit 14070.
Locum Physicians
If you are a locum tenens (as defined in the FPSC Preamble) who works on a temporary basis for a community longitudinal family physician who has submitted 14070 and is away from practice, you can submit 14071.
Physicians with a Focused Practice in Long Term Care
If you are a family physician who has a focused practice in long-term care facilities and is not working as a Community Longitudinal Family Physician, you can submit 14072.
Note: Physicians who work in long-term care AND are a community-based longitudinal family physician do not need to submit 14072.
Physicians working under Alternative Payment/Funding Models
If you are a family physician who provides services under an Alternative Payment/Funding Model, you are not able to submit 14070 or 14071 if your duties would otherwise include provision of this service.
