Description

Hope to Health Clinic is looking to hire a Nurse Practitioner. The start day is flexible and the clinic will accept nurse practitioners whenever they are ready to start.

Remuneration: Starting from $141,494 – $160,312 plus comprehensive health benefits. Dedicated time for non-clinical work. Relocation benefits available.

As part of a team-based approach to full-service family medicine, an interested Nurse Practitioner (NP) would work collaboratively within the team and establish their own patient panel as required by the Ministry. This position can be full-time or part-time and is salaried.

Hope to Health Clinic is excited to add an NP to their team who is committed to working together to deliver primary care to the local community. The role is part of the Primary Care Networks (PCNs) initiative in Vancouver. NPs interested in this opportunity should be willing to work as part of a team and support the development of PCNs in the community.

Please apply or contact us at recruitment@vancouverdivision.com.

NPs will be part of a supportive multi-disciplinary care team that includes family physicians, an RN, LPN, social worker, peer support worker, and MOAs, as well as in house access to other health providers: PT, OT, dietitian, psychologist, and psychiatry consulting services. This opportunity is a 0.3 to 0.5 full time equivalent, with flexible scheduling available. The NP will see about 8 to 10 patients per day. The patient population is mainly complex clients living in the DTES with many co-morbidities, including mental health, addiction, and psychosocial issues.

Protected time for training and QI activities, and for indirect care.

Hope to Health Clinic, a program of the BC Centre for Excellence (BC-CfE), is looking to grow their teams. The clinic provides a team-based care family practice with drop-in available for clinic clients. The Hope to Health currently has 11 exam rooms, and physicians work in a team-based environment in their own work station. The team is well-supported by the BC-CfE with ongoing clinical research and experienced clinical leadership that fosters a great working environment. They provide a range of interdisciplinary primary care services and social supports such as mental health support, addiction treatment, HIV and Hep C treatments, and preventative care to marginalized individuals living in Vancouver’s Downtown Eastside. The clinic does not provide care for children or pregnant women. Hope to Health specializes in an innovative support to substance use disorder; the clinic is quality improvement oriented and offers opportunities to participate in ongoing research.

Other Providers: RN, LPN, peer support worker, social worker, in-house PT, OT, dietitian, phlebotomy, psychologist, psychiatry consulting services

Parking/Bike/Transit information: Limited parking in the back alley, accessible by bus and bike.

Hours: Currently working hours 8:30am-4:30pm, clinic open from 10am-4 pm.
