Re-enrolment Opens January 1

To continue billing under the LFP Payment Model, all physicians and locums must re-enrol annually between Jan 1–Mar 31.

By re-enrolling you confirm that you:

  1. Meet ongoing LFP Eligibility Criteria.
  2. Bill in accordance with the LFP Payment Schedule .

If you miss the March 31 deadline:

  1. Your LFP payments stop until you re-enrol.
  2. You are NOT automatically withdrawn from the model!

Steps

Submit 3 Codes & Optional Transition Code:

  1. LFP Enrolment Code 98000
    • Submit with your MSP Practitioner Number and specific LFP “patient” info (PHN, name, DoB, ICD-9).

    • Clinic 98002 | LTC & Palliative Facility 98003 | Inpatient Facility 98004 | Pregnancy & Newborn Facility 98006
    • Submit with MSP Facility Number for each setting where you bill under the LFP Payment Model.

  3. CLFP Portal Code 14070 (recommended)

  4. If needed: Transition Code 98001 (ends Mar 31, 2026)
    • Submit with the Transition Form ↗ if you don’t yet meet LFP Clinic Non-panel Services ≤30% requirement.

  1. LFP Locum Enrolment Code 98005
    • Submit with your MSP Practitioner Number and specific LFP “patient” info (PHN, name, DoB, ICD-9).
