Date extension for the LFP Transition Process
August 11, 2025

All family physicians enrolled in the LFP Payment Model are required to ensure that Clinic Non-panel Services are no more than 30% of LFP Clinic-based Services.
If needed, you can have a period of transition to meet this requirement. This period of transition has been extended until March 31, 2026.
If you are actively transitioning your clinic-based services to meet this requirement, you are not required to meet the 30% limit on Clinic Non-panel Services when you submit both:
- an LFP Clinic-based Services Transition Code (98001); and
- an LFP Clinic-based Services Transition Form.
The following physicians are not required to submit the 98001 and Transition Form:
- Physicians providing only locum services.
- Physicians practicing in rural communities that were receiving Northern Isolation Allowance (NIA) premiums as of December 15, 2002.
Important Notes
- If you have already submitted both 98001 and the Transition Form, no further action is required at this time.
- If you still require an exception in 2026, you will need to resubmit the LFP Clinic-based Services Transition Code (98001) and LFP Clinic-based Services Transition Form between January 1 and March 31, 2026.
If you have questions about 98001 and the LFP Clinic-based Services Transition Form, please contact FP.billing@doctorsofbc.ca.
