LFP Transition Code & Form Submission — Mark Your Calendar
Family physicians enrolled in the LFP Payment Model are required to ensure that Clinic Non-panel Services do not exceed 30% of total LFP Clinic-based Services.
Transition Period
If you need time to meet this LFP requirement, a clinic transition period is available until March 31, 2026.
To qualify for the transition exception in 2026, you must resubmit the LFP Clinic-based Services Transition Code (98001) and Transition Form between Jan 1–Mar 31, 2026.
During Transition
While you are actively transitioning your clinic-based services to LFP, you are not required to meet the 30% limit on Clinic Non-panel Services if you submit both:
- Transition Code (98001)
- Transition Form ↗
Exemptions
The following physicians do not need to meet the 30% limit on Clinic Non-panel Services and therefore do not need to submit/resubmit the Transition Code and Form:
- Locums
- Rural physicians practicing in communities with Northern Isolation Allowance (NIA) premiums (since Dec 15, 2002).
