All family physicians enrolled in the LFP Payment Model are required to ensure that Clinic Non-panel Services are no more than 30% of LFP Clinic-based Services

If needed, you can have a period of transition to meet this requirement. This period of transition has been extended until March 31, 2026.

If you are actively transitioning your clinic-based services to meet this requirement, you are not required to meet the 30% limit on Clinic Non-panel Services when you submit both:

Important Notes

  • If you have already submitted both 98001 and the Transition Form, no further action is required at this time.
  • If you still require an exception in 2026, you will need to resubmit the LFP Clinic-based Services Transition Code (98001) and LFP Clinic-based Services Transition Form between January 1 and March 31, 2026.
