Indirect patient care includes completion of clinically required forms, reports, and medical certificates of death. Examples include:
- Special Authority Forms
- Plan G Application
- Palliative care Benefits Program Application
- Biosimilars Initiative forms
- Incapability Assessment Report Form
Note: The Teleplan-payable form fee is not payable for physicians enrolled in the LFP Payment Model.
